Laparoscopic management option for common bile duct stone have already used popular in western contury. And compared with endoscopic sphincterectomy and open exploration, it's advantage and positive effect have been reported. But in korea many surgeons do not approach common bile duct store as laparoscopic surgery, because of difficulty of technique and unfamiliar with gasless laparoscopic technique which can solve the difficulty of laparoscopic handling. The author reviews fifty-one cases of common bile duct stone, from 1992 to first half of 1997 for determine it's clinical significance and effectiveness.
Of 51 cases, laparoscopic approach for common bile duct have been improved year by yearly. And approach with gasless technique for common bile duct stone have markedly increased after it's use. For decompression method, about 80% of cases(42) was used with T-tube, and about 14% of cases(7) used ENBD(endo-nasal biliary decompression) catheter, transcystic stone extraction and primary closure for 4%(2). Mean hospital length was 7.6 days and mean operation time was 167.53 min, post operative analgesic use was 1.88 times. In conclusion laparoscopic approach for common bile duct stone is safe and efficacious technique. And especially use of laparoscopic gasless technique for common bile duct stone is recommend.
